Maybe just trying the hammer on over and over again? I don’t have a problem with the low E string but what typically helps me is putting more focus and attention to that hammer on and trying to hammer on straight from above to prevent the string sliding off.
Lol I had the same problem for so long! Except I also have a tendency to vibrato the string off the fretboard
Does it happen when you actually hammer-on or when you pull off from the hammer-on?
Maybe change wrist/thumb position? Then angle of attack for the lowest E will be slightly different and may help with staying on freatboard
And of course with that change, start by slowly repeating then going faster.
Put less force on the low E string unless you absolutely need to (for example, a long legato lick). The low E string is a heavy string so it usually rings longer than others. So no need to put extra pressure. Just gently hammer on it
